Space Shuttle Columbia Investigation Report Released

The Columbia Accident Investigation Board released its report on the accident that destroyed the Columbia space shuttle. While the cause the accident (foam broke from the fuel tank during launch, damaging thermal tiles on the wing, eventually leading to a break up during re-entry) the report also addresses concerns about the Space Shuttle Program's history and culture and the management of NASA.

Watching Satellites Pass Overhead
